It took two lieutenants and two sergeants to determine the cause of an "industrial accident" - brushes at the carwash broke off a mirror on an unmarked Salt Lake police patrol car.

According to a police report, Records Division Lt. John Schroen took a shop vehicle to the carwash at 627 S. West Temple for an oil change and cleaning. While the car was going through the wash, a sideview mirror got knocked off.Schroen called Lt. Tom Brown, a watch commander who must examine all mishaps involving city property. Two traffic sergeants also responded and quickly determined the extent of damage.

The right-side mirror, worth about $50, was found inside the carwash at the time of the industrial accident, the report said.
