A West Valley man died when his vehicle collided head-on with another vehicle after he possibly lost consciousness while driving west on 35th South Wednesday afternoon, a police report says.

James Veliotis, 56, was pronounced dead at Pioneer Valley Hospital about 4:30 p.m., the West Valley police report said.The driver of the second car, Lee Cook, 18, also of West Valley City, was treated at Pioneer Valley then released, a hospital spokeswoman said.

Cook said he could not see the driver of the other car just prior to the collision, the report said. Other witnesses told police they saw Veliotis slumped over the steering wheel of the car before the collision, the report said.
