Phyllis Worthen Kolovich, loving wife, mother, sister, aunt and friend, passed away February 2, 1991, after a courageous battle with cancer.

She was born April 18, 1933 in Castle Dale, Utah, to Joseph Archie and Leah Jensen Worthen. Married Michael Kolovich on April 18, 1957 in Price, Utah. Their marriage was later solemnized on March 4, 1969 in the Salt Lake LDS Temple. She was raised by her grandparents Jode and Minnie Worthen in Ferron, Utah. Active member of the LDS Church. Served as Relief Society President and othercallings in the church. Phyllis had a tremendous love for life and her family and will be deeply missed by all.Survived by her husband, Michael; two daughters, Julie andKaren; one son Kent; sister Anna Louise (husband LaVar) Weight, all of Salt Lake; two sisters/aunts, Iris Sommer, Wyoming and Sodonia Jolley, New Mexico.

Funeral services will be held Wednesday February 6, 1991, at 12:00 noon at the Garden Heights Ward, 2220 Fisher Lane (2935 South), with Bishop T. Eugene King officiatng. Friends may call Tuesday evening from 6 to 8 PM at the Holbrook Funeral Chapel, 3251 South 2300 East, and at the church on Wednesday for one hour prior to services.
