A West Valley man was killed Wednesday when a heavy-duty forklift ran over him at the construction site of the new Utah Jazz arena.

Killed in the accident was:Jeff Fitzwater, 35, West Valley City.

He was pronounced dead at the scene of the 1 p.m. accident after suffering massive head injuries. He was an employee of the O.C. Sahara Construction Co.

Fitzwater was apparently standing and talking with someone and may not have noticed the forklift backing up.

"He either got snagged on the tire or slipped and fell under the wheel," said Salt Lake Police Lt. Ken Thirsk.

Thirsk said the ground was icy and slick in the area.

The forklift driver's vision may have been obstructed by other equipment, police said. Although the heavy equipment's back-up beeper was sounding, Thirsk said, there are several machines with beepers at the site, and most workers likely don't notice them because they are constantly sounding.

Ironically, the workers had attended a safety meeting earlier that morning.

Last month, another construction worker at the site was hospitalized after he fell from a beam. Mike Cordova, 20, of Salt Lake City, suffered broken bones and head injuries after falling about 35 feet and landing on concrete below.
