A Holladay dentist accused of fraudulently billing insurance companies has had his license to practice placed on five years probation, an order said.
On two occasions, Dr. Kent S. Lewis submitted claims for services not rendered on the dates specified, the order said. The services were performed earlier. But clerical error and misinformation from a patient caused the insurance companies to reject the claims, the order said."His proper recourse should have been to either obtain payment from the patients or waive his charges under the circumstances," said the order from the Division of Occupational and Professional Licensing.
Calling Lewis' conduct "seriously disconcerting," the division has required him to perform 200 hours of community service.
