I believe the location of the accident was Federal Highway 89", not "State Road 89".
This is highly confusing and happens all the time. The road that runs along the East bench from Lagoon through Ogden (Washington Blvd) and then continues on as "fruit way" in Willard and Perry is a federal highway and is designated 89 by number.
I'm not sure if there is a state highway 89 or not.
SR 89 is a state road! Hence the "SR". Check your map!
