Looked that robot accident up. It was a robotic arm designed to grab supplies from a supply depository, they had tweaked it to go faster for efficiencies sake. The man mentioned was picking up supplies in the same spot and was fatally struck in the head.
Another more interesting case was an incident in 1981 that many thought to be the first human casualty. An engineer at Kawasaki was working on a broken robot that was not completely turned off. He accidentaly triggered its arm which pushed him into a grinder.
