Prices rise on birth, death certificates

The cost of birth and death certificates just went up.

The Utah Department of Health Office of Vital Records and Statistics increased the fees July 1. The fee increases were passed by the Utah Legislature to balance the health budget.

Birth certificate cost went from $12 to $15 and the death certificate fee changed from $9 to $14. The fee for duplicates of both certificates after the initial certificate fee is paid went from $5 to $8.

The new fee schedule, application forms and an online ordering service can be accessed online at www.health.utah.gov/vitalrecords, or at the service window in the Martha Hughes Cannon Health Building at 288 N. 1460 West in Salt Lake City. The mailing address for the Office of Vital Records and Statistics is P.O. Box 141012, Salt Lake City, UT 84114-1012.
