WASHINGTON, D.C. — Utah will receive $137,193 in grant money to help seniors, caregivers and people with disabilities find their way through new health and long-term care options as part of the Affordable Care Act's $68 million grants program.
Health and Human Services Secretary Kathleen Sebelius announced the grants Monday. She said the funds were specifically available to help families understand their Medicare and Medicaid benefits, including coverage for preventive services, explore options for long-term care including services that help individuals stay in their homes and help those making the transition from nursing or rehabilitation facilities back into the community.
