Uninsured Utahns can apply for health insurance through the Utah Department of Health's Primary Care Network between Nov. 28 and Dec. 9.
To qualify, applicants must meet income requirements and be between 19 and 64 years old, have no other access to health insurance and cannot be a full-time student.
PCN covers a range of preventive health care services, including dental care, mammograms and Pap smears, birth control methods, up to four prescriptions per month and immunizations.
The cost is $50 per month, plus co-payments.
Beginning Nov. 28, call 1-888-222-2542 or visit www.health.utah.gov/pcn to apply.
