S.L. County 10-year plan to end homelessness

Highlights:

• Develop discharge plans for the homeless from jail, hospital or treatment center

• Increase public-sector funding for affordable housing

• Improve homeless outreach to reduce homeless persons on the street by 95 percent

• Rapidly re-house people within 90 days of becoming homeless

• Create a database of existing housing for homeless

• Increase the supply of permanent supportive housing and rentals
